摘要:
目的 探讨悬吊核心训练与核心肌群训练对减轻腰椎间盘突出症患者疼痛及功能障碍的疗效。方法 前瞻性收集2018年1月至2018年6月河北医科大学第三医院康复二科收治的30例腰椎间盘突出症患者,随机分为两组,每组15例,两组均给予常规物理治疗:超短波治疗与中频脉冲治疗。治疗组为悬吊核心训练组,对照组为核心肌群训练组,治疗组给予悬吊训练,对照组给予核心肌群训练,每日一次,一周5次,连续4 w。通过视觉模拟疼痛评分法(VAS)、日本骨科协会评估治疗分数(JOA),Oswestry功能障碍指数问卷(ODI)进行评定。分别与治疗前、治疗结束时和治疗结束8 w后对患者进行评定。结果 治疗结束时及治疗结束后8 w,悬吊核心训练组及核心肌群训练组的VAS评分较治疗前显著降低,差异有统计学意义(F=765.103,P<0.05);JOA评分较治疗前显著提高,差异有统计学意义(F=614.399,P<0.05);ODI评分较治疗前明显下降,差异有统计学意义(F=617.315,P<0.05)。治疗结束时及治疗结束后8 w,悬吊核心训练组VAS评分低于核心肌群训练组,差异有统计学显著性意义(F=18.568,P<0.05);JOA评分高于核心肌群训练组,差异有统计学意义(F=5.284,P<0.05);ODI评分低于核心肌群训练组,差异有统计学意义(F=4.819,P<0.05)。结论 悬吊训练与核心肌群训练均能缓解腰椎间盘突出症的疼痛及减轻功能障碍,而悬吊训练组改善的效果优于核心肌群训练组。
Abstract:
Objective To explore the effect of sling exercise therapy and core stability exercise therapy to alleviate pain and improve lumbar function on patients with lumbar discherniation. Methods Prospectively collected thirty lumbar disc herniation patients were divided randomly into two exercise groups,the sling exercise therapy group (n=15) and the core stability exercise therapy group (n=15). Both groups received conventional physical therapy: ultrashort wave therapy and intermediate frequency pulse therapy. The treatment group was sling exercise therapy group, and the control group was core stability exercise therapy group. The treatment group was given sling training, and the control group was given core muscle training, once a day, 5 times a week for a continuous period of 4 weeks. The effects were assessed through visual analogue scale (VAS), Japanese orthopaedic association (JOA) and Oswestry disability index (ODI). The assessments were performed at the beginning, the end and 8 weeks after treatment. Results At the end and 8 weeks after treatment, VAS scores (F=765.103, P<0.05) and ODI scores (F=617.315, P<0.05) of two groups were lower than that at the beginning of treatment, the JOA scores(F=614.399, P<0.05) was higher than the beginning of the treatment. At the end and 8 weeks after treatment, the VAS scores of the sling exercise therapy group was lower than the core stability exercise therapy (F=18.568, P<0.05), the JOA scores of the sling exercise therapy group was higher than the core stability exercise therapy (F=5.284, P<0.05), the ODI scores of the sling exercise therapy group was lower than the core stability exercise therapy (F=4.819, P<0.05), the difference is significantly. Conclusion Both of the sling exercise therapy and the core stability exercise therapy could reduce pain and improve lumbar function; and the sling exercise therapy was more effective and longer lasting than the core stability exercise therapy for lumbar disc herniation treatment.
