摘要:
目的 探讨降钙素受体基因(calcitonin receptor,CTR)C1377T 基因多态性与老年 男性人群骨质疏松的相关性。方法 前瞻性收集2014 年8 月至2015 年8 月在河北医科大学第三医 院门诊就诊及住院患者中无亲缘关系且符合纳入及排除标准的老年男性220 例,按照骨质疏松诊断 标准分为骨质疏松症组(n=116)和非骨质疏松症组(n=104)。用聚合酶链反应限制性片段长度 多态性技术检测其CTR 基因多态性,并测量骨质疏松症组患者的跟骨定量超声参数:超声振幅衰 减(broadband ultrasound attenuation ,BUA)、超声声速(speed of sound,SOS) 及刚度(systems technologies inc,STI)。通过比较两组人群中CTR 基因型频率,并分析CTR 基因型与跟骨定量 超声参数之间的关系,探讨CTR 基因多态性与老年男性骨质疏松症的相互关系。结果 骨质疏松 症组CTR 基因型CC 型占81.00%,CT 型占19.00%;非骨质疏松症组CC 型占7 2 . 1 0 % , CT 型占27.90%,两组比较差异无统计学意义(χ2=2.450,P=0.118);骨质疏松症组C 等位基因 占90.52%,T 等位基因占9.48%;非骨质疏松症组C 等位基因占86.06%,T 等位基因占13.94%,两 组比较差异无统计学意义(χ2=2.128,P=0.145)。但骨质疏松症患者CT 基因型组的BUA、SOS 和 STI 均明显高于CC 基因型组,差异有统计学意义(P < 0.05)。结论  CTR 基因多态性不是影响 骨质疏松发病的重要因素,但CTR 基因型和定量超声参数所反映的骨密度、骨强度和骨的结构等指 标均有一定相关性。
Abstract:
 Objective To investigate the relationship between the C1377T polymorphisms of calcitonin receptor (CTR) gene and the incidence of osteoporosis in Chinese elderly male population. Methods A total of 220 elderly male outpatients who visited the clinic of the Third Hospital of Hebei Medical University between August 2014 and August 2015 were recruited, and divided into osteoporosis group (n=116) and non-osteoporosis group (n=104) according to the diagnostic criteria of osteoporosis. The genotype of CTR gene was examined by polymerase chain reaction-restriction fragment length polymorphism (PCR-RELP). Broadband ultrasound attenuation (BUA), speed of sound (SOS) and systems technologies inc (STI) of osteoporosis patients were determined by UBIS3000 quantitative ultrasound (QUS). Results Genotypes CC and CT of CTR gene of osteoporosis group were 81.00% and 19.00%, the genotypes in normal group were 72.10% and 27.90% respectively, and there were no significant differences in the distribution of genotypes between these two groups (χ2=2.450, P=0.118). Three quantitative ultrasound indexes in group of genotype CT were higher than those in genotype CC group. Conclusion  CTR gene polymorphism may not be the important factors influencing the incidence of osteoporosis. But, CTR gene is connected with bone deity, bone strength and bone structure.
