摘要:
目的 分析膝关节镜下治疗老年骨关节炎合并腘窝囊肿的临床疗效。方法 回顾性分析2013年7月至2015年12月间吉林医药学院附属医院收治的老年膝关节骨关节炎合并腘窝囊肿的患者50例，根据手术方式分为关节镜组（n=25）和开放手术组（n=25），随访并比较两组患者手术时间、术中出血量、术后恢复率和囊肿复发率情况。结果 所有患者均获得随访，随访时间为8～12个月，平均（10.2±2.1）个月，随访率为100%。关节镜组患者手术时间[（30.2±3.4）min]长于开放手术组[（19.9±2.3）min]，术中出血量[（4.5±1.8）ml]少于开放手术组[（38.2±8.1）ml]，差异均有统计学意义（t=-12.67，t=20.31，P＜0.05），术后0级恢复率关节镜组为88%，开放手术组为80%，囊肿复发率关节镜组为4.0%，开放手术组为8.0%，差异无统计学意义。结论 关节镜治疗老年膝关节骨关节炎合并腘窝囊肿患者创伤小，术中出血少，但在提高患者术后0级恢复率、降低复发率等方面仍需大样本的研究进行检验。
Abstract:
Objective To analyze the effect of arthroscope in the treatment of elderly osteoarthritis combined with popliteal cyst. Methods A retrospective analysis was performed in 50 elderly patients of osteoarthritis combined popliteal cyst from July 2013 to December 2015 in the Affiliated Hospital of Jilin Medicine University. They were divided into two groups according to different operation methods: arthroscopy group (n=25), open surgery group (n=25). Operative time, blood loss, postoperative recovery rate and cyst recurrence were recorded and compared. Results All patients received an average of 10.2±2.1 months (range, 8-12 months) of followed-up, and the rate of follow-up was 100%. The arthroscopy group 30.2±3.4 min, 4.5±1.8 ml had longer surgery time and lower blood loss than open surgery group (19.9±2.3 min, 38.2±8.1 ml), with statistical significance (t=-12.67, t=20.31, P<0.05), level 0 postoperative recovery rate in arthroscopy group was higher than the open surgery group (88% vs 80%), cyst recurrence rate was lower than the open surgery group (8.0% vs 4.0%), with no statistical significance. Conclusion Arthroscope is a less injury treatment for elderly patients with osteoarthritis combined with popliteal cyst, but the effectiveness need more solid researches to identify.
