摘要:
目的 比较腓骨近端截骨术与单髁关节置换术(unicompartmental knee arthroplasty,UKA)治疗膝关节内侧单间室骨关节炎的临床疗效。方法 回顾性分析2015 年1 月至2015 年10 月济南军区总医院收治的符合纳入及排除标准的膝关节内侧间室病变的28 例(36 膝)患者,根据手术方式分为腓骨截骨组及UKA组。腓骨截骨组患者13 例(17 膝),男性3 例,女性10 例,年龄41 ~ 68 岁,平均(53±6)岁;UKA组患者15 例(19 膝),男性5 例,女性10 例,年龄54 ~ 73 岁,平均(62±8)岁。随访并比较两组患者的手术时间、术中出血量、住院时间、医疗费用及手术前后美国特种外科医院(hospital for special surgery,HSS)膝关节评分、疼痛视觉模拟评分(visual analogue scale,VAS)等。结果 28 例患者均获得完整随访,随访时间为2 ~ 11 个月,平均8 个月。与UKA 组相比,腓骨截骨组患者的手术时间、术中出血量、住院时间、医疗费用均较低,差异有统计学意义(P < 0.05)。组内比较:两组患者的术后HSS 评分、VAS 评分及膝关节活动度均较术前有明显好转,差异有统计学意义(P < 0.05)。组间比较:术前、术后HSS 评分、VAS 评分以及术后膝关节活动度差异均无统计学意义(P > 0.05)。结论 腓骨近端截骨与UKA 治疗膝关节内侧单间室骨关节炎均具有较好早期疗效,但腓骨近端截骨术具有操作简单、创伤小、恢复快、费用低等优点。
Abstract:
Objective To explore the outcome of proximal fibular osteotomy andunicompartmental knee arthroplasty (UKA) for medial compartment osteoarthritis of the knee. MethodsThirty-six knees of 28 cases (36 knees) with medial compartmental osteoarthritis of the knee fromJanuary 2015 to October 2015 were reviewed retrospectively. In proximal fibular osteotomy group therewere 13 patients (17 knees), 3 males and 10 females, aged from 41 to 68 years, with an average age of(53±6) years. In UKA group there were 15 patients (19 knees), 5 males and 10 females, aged from 54to 73 years, with an average age of (62±8) years. The operation time, blood loss, hospitalization time,medical expenses, the visual analogue scale (VAS) and the hospital for special surgery (HSS) knee scorewere compared between the two groups. Results 28 patients were followed up from 2 to 11 months,with an average of 8 months. Compared with the UKA groups, the operation time, the amount of bloodloss, hospitalization time and medical expenses of the proximal fibular osteotomy group were lower, thedifference was statistically significant (P<0.05). Compared within the group, the HSS knee score andVAS were significantly improved, the difference was statistically significant (P<0.05). Compared between the two groups, there was no significant difference in HSS knee score, VAS and postoperative knee jointactivity (P>0.05). Conclusion Proximal fibular osteotomy and UKA have good effect in treating medialcompartment knee osteoarthritis, but proximal fibular osteotomy has the advantages of small trauma, fastrecovery and lower cost.
