摘要:
股骨转子间骨折是全球范围内的重大公共卫生问题之一。身体条件可耐受的患者应首选内固定手术治疗,以实现早期康复并预防卧床相关的并发症。骨折复位质量决定了手术效果,因而复位技术的进展受到骨科医生的持续关注。目前,前内侧皮质支撑复位技术经过近8年的发展,已在国际上获得认可,但新出现的问题仍有待解决:基础研究数量与质量有待提高;正性支撑与中性支撑孰优孰更优;前侧支撑与内侧支撑孰更重要;中性支撑与解剖复位的异同;Chang复位质量标准与Baumgaertner复位质量标准的对比。本文系统回顾了近年来的基础研究与临床研究,总结了相关的新进展、新争议,以此为前内侧皮质支撑复位理论的临床应用及后续研究提供参考。
Abstract:
The trochanteric hip fracture is one of the most serious public health issues. Surgical intervention achieves early mobilization and prevents secondary complications. In recent years, the positive anteromedial cortical support, one non-anatomical fracture reduction technique, acquired wide recognition but with new controversial issues: the lack of experimental studies; the comparison between positive and neutral cortical support; the comparison between anterior and medial cortical support; the difference between neutral cortical support and anatomical reduction; and the comparison between Chang and Baumgaertner reduction quality criteria. This review provides an overview of related reports of clinical study, finite element analysis as well as biomechanical testing, and summarizes the research progress and debate in this field.
