摘要:
目的 对比分析我国南方和北方地区60岁以上老年股骨颈骨折的流行病学特点。方法 利用医学影像计算机存档与传输系统及病案查询系统,回顾性分析2010年1月至2011年12月间,我国南方和北方地区21所医院收治的60岁以上的股骨颈骨折患者资料。其中,北京市、甘肃省、吉林省、辽宁省和宁夏回族自治区12所医院为北方组,广东省、广西省、湖南省、贵州省和云南省9所医院为南方组。对比分析两组患者的性别、年龄、侧别、各年龄段骨折例数及骨折Garden分型等。结果 共收集到60岁以上的股骨颈骨折病例1 945例,其中南方组1 039例,男性361例(34.7%),女性678例(65.3%);北方组906例,男性320例(35.3%),女性586例(64.7%)。年龄构成方面,南方组和北方组均为76~80岁年龄段所占的比例最高,分别为23.6%(245/1 039)和23.5%(213/906),南方组86岁以上的年龄段占比例最低,为9.5%(99/1 039);北方组61~65岁年龄段占比最低,为11.5%(104/906)。南方组和北方组各个年龄段均是女性患者多于男性。南方组的总体高发骨折类型为GardenⅢ型,占77.7%(807/1 039),北方组的总体高发骨折类型同样为GardenⅢ型,为65.0%(589/906),占比较南方组低(c2=4.147,P<0.05)。南北方各骨折类型都是女性多于男性,但GardenIV型北方组女性占比高于南方组(c2=3.902,P<0.05)。结论 两个地区均为76~80年龄段的患者所占的比例最高,并且各个年龄阶段女性患者均多于男性。两个地区的骨折构成比最高的类型均是GardenⅢ型,但南方地区该型所占比例更高。
Abstract:
Objective To compare and investigate the epidemiological characteristics of geriatric femoral neck fractures between the south and north area of China. Methods The data of pediatric femoral neck fractures treated in 9 hospitals in south China and 12 hospitals in north China between January 2010 and December 2011 were collected through the PACS system and medical records inquiry system. South group includes Guangdong, Guangxi, Hunan, Guizhou, Yunnan Province and north area group includes Beijing, Liaoning, Jilin, Shanxi, Gansu and Ningxia Province. Analytic items included gender,age and Garden type were compared and analyzed between two groups. Results A total of 1,945 cases were divided into south group (1,039 cases) and north group (906 cases), respectively. There were 361 (34.7%) males and 678 (65.3%) females in south group, while 320 (35.3%) males and 586 (64.7%) females in north group. Age distribution showed that the peak range was 76-80 years in both groups with proportion of 23.6% (245/1,039) in south group and 23.5% (213/906) in north group, while the valley range was ≥86 years old in south group with proportion of 9.5% (99/1,039) and 61-65 years old in north group with proportion of 11.5% (104/906). The high incidence of fracture types is Garden Ⅲ in both groups with proportion of 77.7% (807/1,039) in south group and 65.0% (589/906) in north group. Conclusions The results show that both regions had the highest proportion of patients in age 76 to 80, with more female than male at all age groups. The high incidence of fracture type is Garden Ⅲ in both groups, but the south group has a higher proportion.
