摘要:
目的　观察围术期抑郁情绪对老年骨科手术患者外周血单核细胞组织因子（TF）表达及血小板功能的影响。 方法　前瞻性收集 2012 年 7 月至 2014 年 6 月河北医科大学第三医院行腰椎管狭窄腰后路全椎板减压植骨融合内固定术的老年患者（≥ 60 岁） 100 例，根据术前 1 d 的汉密尔顿抑郁量表（HAMD）和汉密尔顿焦虑量表（HAMA）评分分为：轻度抑郁组（A 组），重度抑郁治疗组（B 组），重度抑郁非治疗组（C 组），抑郁焦虑共病组（D 组）。 B 组患者于术日始每天进行心理辅助治疗，共 7 d。各组患者于术前 1 d 及术后第 7 天取静脉血测定单核细胞 TF 表达、血小板功能 [ 指标达峰值时间（TP）、最大凝血标记值（MCS） ] 及进行 HAMD 评分。 结果 （1） B 组患者手术前后的 HAMD 评分差值较 A、 C 和 D 组低（P ＜ 0.05）；（2） B 组患者手术前后的 TF 荧光强度差值低于 A 组， A 组低于 C 及 D 组（P ＜ 0.05），抑郁评分与 TF 荧光强度的变化呈显著性正相关（r=0.431， P ＜ 0.05）；（3） B 组患者手术前后的 TP 差值低于 A、 C 和 D 组（P ＜ 0.05），抑郁评分与 TP 的变化呈显著性负相关（r=?0.257， P=0.01）；（4） A、 B 组的 MCS 术后、术前差值低于 C 和 D 组（P ＜ 0.05），抑郁评分与 MCS 变化呈显著性正相关 （r=0.219， P=0.029）。 结论　老年骨科患者围手术期抑郁情绪障碍将导致外周血单核细胞 TF 表达增加，及血小板功能的增强。
Abstract:
Objective To investigate the impact of depressive disorder on tissue factor expression of monocytes and platelet function in elderly patients undergoing orthopaedic surgery. Methods One hundred patients ( ≥ 60 yrs) with lumbar spinal stenosis from July 2012 to June 2014 were divided into 4 groups according to the hamilton depression scale (HAMD) and hamilton anxiety scale (HAMA): Mild depression group (Group A), Depression and Therapy group (Group B), Depression and Non-therapy group (Group C), Depression and Anxiety group (Group D). Daily psychological therapy was carried out in Group B from the day of operation to the seventh day after operation. The day before operation and the seventh day after operation, venous blood was drew for the detection of monocyte TF expression and platelet function (time to peak, TP; maximal clot signal, MCS), and HAMD evaluation. Results ① The pre- and postoperative difference of HAMD in Group B was lower than Group A, C and D (P<0.05). ② The pre- and post-operative difference of TF expression of Group B was lower than Group A, and a decresing order was Group A
