摘要:
随着我国老龄化进程不断加速，老年人成为社会人口中增长最快的部分，并且相较其他年龄组接受手术更频繁。老年人身体机能差，活动能力受限，对伤害性刺激抵御能力下降，而且重要脏器储备能力下降，从而增加了老年人的手术频率和风险。骨科手术治疗创伤较大，患者术后常出现剧烈疼痛，有效的术后镇痛直接影响患者术后早期康复和活动，因此完善的术后镇痛对骨科手术患者的恢复起到关键性作用。老年骨科患者术后疼痛的处理可能会因多种因素而变得复杂，包括年龄、疾病相关的生理、疾病相关的药物以及药物与药物相互作用等，而且术后疼痛控制不足与老年患者的不良预后密切相关，因此老年骨科患者术后镇痛方案的制定就尤为重要。本文就老年人的生理特点及各种常用术后镇痛方式的新进展进行了综述。
Abstract:
As China’s aging process continues to accelerate, older people have become the fastest growing segment of the social population and are undergoing surgery more frequently than other age groups. The elderly have poor physical function, limited mobility, reduced resistance to noxious stimuli, and decreased vital organ reserve capacity, thereby increasing the frequency and risk of surgery for the elderly. Orthopedic surgery is more traumatic, and patients often experience severe pain after surgery. Effective postoperative analgesia directly affects early postoperative rehabilitation and activities. Therefore, perfect postoperative analgesia plays a key role in the recovery of orthopedic surgery patients. Postoperative pain management in elderly orthopedic patients may be complicated by a variety of factors, including age, disease-related physiology, disease-related drugs, and drug-drug interactions, and inadequate postoperative pain management and poor elderly patients. The prognosis is closely related, so the development of postoperative analgesia in elderly orthopedic patients is particularly important. This article reviews the physiological characteristics of the elderly and the new developments in various commonly used postoperative analgesia.
